Botriocefalosi
Che cos’è la botriocefalosi?
La botriocefalosi è una parassitosi causata dalla tenia, diphyllobothrium latum, contratta dall’ingestione di pesce crudo o insufficientemente cotto. Il diphyllobothrium latum rappresenta il più grande verme patogeno in grado di parassitare l’uomo (può raggiungere la lunghezza di 15 metri). La botriocefalosi è solitamente asintomatica.
Come si contrae?
La botriocefalosi viene contratta attraverso l’ingestione di pesce d’acqua dolce crudo o poco cotto, le cui carni siano parassitate dalle larve di biotrocefalo; presenta perciò una maggiore diffusione nelle aree ricche di laghi e corsi d’acqua, in cui è più abituale la consumazione di pesce d’acqua dolce Giappone, Stati Uniti, Nord Europa; anche alcuni Laghi Alpini italiani, quali il Lario, sono interessati da diphyllobothrium latum.
Quali sono i sintomi della botriocefalosi ?
I sintomi principali della botriocefalosi sono:
- Dolori addominali
- Bulimia o anoressia
- Perdita di peso
- Anemia perniciosa, con glossite e sindrome neurologica
- Forme asintomatiche frequenti.
La presenza a livello intestinale dei vermi adulti di Diphyllobothrium latum causa un depauperamento di vitamina B12, che il parassita utilizza per il proprio metabolismo: nelle infestazioni massive, accanto a turbe della canalizzazione è dunque frequente il riscontro di un’anemia megaloblastica su base competitiva.
Diagnosi
La botriocefalosi viene diagnostica attraverso l’evidenziazione delle caratteristiche uova all’esame parassitologico delle feci.
Terapia
La terapia si basa sull’utilizzo del praziquantel o di niclosamide, cui è utile associare acido folico e vitamina B12 nei quadri caratterizzati da anemia.
Prevenzione
La prevenzione si basa sulla corretta cottura dei pesci potenziali veicoli di infezione.
