Cataratta congenita
La cataratta congenita si manifesta con opacità del cristallino presente alla nascita o che si sviluppa nei primi mesi di vita. Rappresenta dal 10 al 40% la causa di cecità nell’infanzia. Colpisce lo 0,4 % dei neonati. E’ un’importante causa di ambliopia e strabismo (se unilaterale) nistagmo (se bilaterale).
Problemi visivi in relazione alle dimensioni, sede e densità
dell’opacizzazione .
Forme monolaterali maggiormente correlata da ambliopia.
La cataratta congenita può essere capsulare, capsulo-lenticolare.
La cataratta polare anteriore e quella nucleare sono generalmente statiche, anche se possono raramente progredire.
Le cataratte che in genere progrediscono sono la lamellare, la strutturale e la sottocapsulare. Non tutte le cataratte sono visivamente significative. Se un’ opacità lenticolare riguarda l’asse visivo, di solito è considerata visivamente significativa e richiede la rimozione.
Eziologia
Idiopatica
Infezioni intrauterine: rosolia, morbillo, varicella, citomegalovirus, herpes simplex, herpes zoster, poliomelite, influenza, virus di Epstein-barr, sifilide, toxoplasmosi.
Prematurità e sofferenza feto-neonatale
Ereditarietà: autosomica dominante, autosomica recessiva,
X-linked.
Malattie metaboliche: galattosemia, diabete, ipocalcemia,
morbo di Wilson, morbo di Fabry, morbo di Refsum.
Anomalie oculari: aniridia e anomalie del segmento anteriore, persistenza del vitreo primitivo iperplastico, microftalmo, retinopatia del prematuro.
Alterazioni cromosomiche: trisomia 21, 13, 18, sindrome di Turner.
Malattia a carattere sistemico: malformazioni cranio-facciali, malattie scheletriche e digitali, anomalie del SNC, sindromi ad interessamento renale.
Malattie muscolari: distrofia miotonica.
Malattie dermatologiche: sindrome di Cockayne, dermatite atopica, ittiosi.
Classificazione
In base alla localizzazione anatomica dell’ opacità:
1.Totale
2. Polare (anteriore e posteriore): interessa solo la capsula oppure
si approfondisce nel parenchima lenticolare.
3. Pulverulenta: opacità puntiformi localizzate intorno al nucleo embrionale o nel nucleo embrionale (centrale) bilaterale.
4. Zonulare
a. Lamellare: questo è il tipo più comune di cataratta congenita.
Tali casi sono di solito bilaterali e simmetrici e la densità di opacizzazione può variare notevolmente.
b. Stellare: colpisce la regione delle suture.
c. Coronarica: è radiale, forma una discreta opacità che si trova nella corteccia, assomiglia alla cima di una corona. A causa della loro posizione periferica, non diminuisce l’acuità visiva.
d. Nucleare: di solito bilaterale, coinvolge il nucleo embrionale
o fetale, può mostrare una varietà molto ampia di gravità.

Trattamento
Nei bambini affetti da una cataratta bilaterale o unilaterale, le indicazioni per la chirurgia e il rapporto rischio-beneficio sono più complessi che per un paziente adulto. Nei bambini, i tempi di intervento chirurgico possono essere dettati dal grado di maturazione del sistema visivo.
Può essere necessario eseguire un intervento chirurgico in modo tempestivo per facilitare il trattamento efficace dell’ambliopia.
Cataratta densa bilaterale: rimozione nel più breve tempo possibile.
Cataratta bilaterale: breve intervallo di tempo tra i due occhi per prevenire l’ambliopia del secondo occhio.
Cataratta parziale: valutazione dell’ eventuale impedimento visivo con eventuale operazione chirurgica.
Intervento
Il trattamento consiste nell’asportazione del cristallino o nel posizionamento di una nuova lente intraoculare (IOL).
L’ intervento maggiormente eseguito è la facoemulsificazione (ultrasuoni). Si tratta di un intervento di estrazione extracapsulare del cristallino in quanto la capsula posteriore viene lasciata in situ.
Più raramente viene eseguito l’intervento di estrazione intracapsulare del cristallino.
