Il linfoma primitivo del mediastino si presenta come una massa nella parte anteriore del mediastino
Linfoma primitivo del mediastino

 

 

Il linfoma  primitivo del mediastino (Primary mediastinal b-cell lymphoma- PMBCL) rappresenta meno del 3% di tutti i linfomi non-Hodgkin. Il linfoma primitivo del mediastino colpisce prevalentemente la popolazione giovane adulta (picco di incidenza tra i 30 e i 40 anni) prediligendo il sesso femminile, con un rapporto maschi/femmine 1:2.

La diagnosi di linfoma primitivo del mediastino si basa sull’integrazione di dati clinici, morfologici, immunofenotipici, genetici e molecolari, questi ultimi valutati su tessuto fresco ottenuto dalla biopsia della massa adenopatica. Tali dati permettono di distinguere il  linfoma primitivo del mediastino dal  linfoma Hodgkin variante classica e dal linfoma non-Hodgkin diffuso a grandi cellule, con i quali entra in diagnosi differenziale.


Manifestazioni cliniche 

 

Nel 70% dei casi si presenta come una massa tumorale bulky, localizzata nel mediastino anteriore, a crescita rapida e progressiva e ad estensione locale con possibile infiltrazione di parenchima polmonare, gabbia toracica, pleura e pericardio; in un terzo dei casi è presente versamento pleurico e/o pericardico. La neoformazione mediastinica può causare sintomi da compressione locale, come:

  • Tosse secca
  • Dispnea
  • Disfagia
  • Disfonia
  • Segni di ostruzione cavale

Dal 30 al 50% dei pazienti si presentano infatti con sindrome mediastinica (sindrome della vena cava superiore) caratterizzata da:

  • Edema “a mantellina”
  • Turgore delle vene giugulari
  • Edema mammario
  • Circolo venoso superficiale prominente

A volte l’esordio clinico è dirompente tanto da costituire un’ emergenza ematologica. In meno del 20% dei pazienti sono presenti all’esordio sintomi sistemici quali:

  • Febbre o  febbricola
  • Calo ponderale
  • Sudorazioni profuse soprattutto notturne
  • Prurito sine materia (senza causa)

Data la precocità dei sintomi compressivi, la malattia viene solitamente diagnosticata quando è ancora localizzata esclusivamente a livello toracico; infatti nonostante l’invasività locale, la diffusione a distanza è rara nelle fasi iniziali. L’infiltrazione del midollo osseo è rara  come negli altri linfomi aggressivi.


Esami strumentali

Nell’ 80% dei casi il linfoma primitivo del mediastino viene diagnosticato in stadio I-II per l’interessamento sopradiaframmatico, secondo il sistema Ann-Arbor. Per quanto concerne le metodiche stadiative del  linfoma primitivo del mediastino, esse comprendono la tomografia assiale computerizzata (TC) con mezzo di contrasto iodato e la tomografia per emissione di positroni (PET) total body, da associare alla biopsia midollare con agoaspirato midollare.

 

Prognosi

A causa della giovane età di insorgenza e dal fatto che il linfoma primitivo del mediastino è spesso unicamente confinato al mediastino: fanno sì che oltre il 50% dei pazienti abbia, alla diagnosi, un IP score basso. Pertanto, alcuni autori hanno cercato di individuare dei fattori di rischio specifici del linfoma primitivo del mediastino: secondo l’esperienza della British Columbia, valori di LDH aumentato di 2 volte rispetto al limite superiore dei valori normali, un’età superiore ai 40 anni e un performance status pari o  superiore a 2 sono correlati con una ridotta sopravvivenza; mentre secondo l’esperienza dell’interno dell’International Extranodal Lynphoma Study Group sono fattori prognostici negativi il sesso maschile, un ridotto performance status ed una malattia di esordi in stadio avanzato.


Terapia

 

La terapia del linfoma primitivo del mediastino varia da schemi di chemioterapia di prima generazione, basati sull’impiego della ciclofosfamide e dell’ antraciclina, a regimi di terza generazione, maggiormente aggressivi e a cadenza più frequente, caratterizzati dall’impiego continuo dello steroide. L’associazione dell’anticorpo monoclonale Rituximab alla chemioterapia ha migliorato la sopravvivenza libera da malattia e la percentuale di risposte. Il ruolo della radioterapia nel linfoma primitivo è tuttora oggetto di controversia.
