Che cos’è il morbo di buerger?
Il morbo di buerger o tromboangoite obliterante è un processo infiammatorio ostruente delle arterie di medio e piccolo calibro. Si tratta di una malattia che colpisce di preferenza le arterie periferiche (gamba, visceri) di soggetti in prevalenza giovani ( dai 25 ai 40 anni) di sesso maschile.
Quali sono le cause del morbo di buerger?
La causa della malattia non è ancora stata accertata, ma sono state fatte considerazioni ben precise. E’ indubbio che la malattia è in rapporto con il tabagismo (intossicazione cronica da tabacco) giacchè insorge frequentemente in forti fumatori e presenta una certa regressione con la sospensione del fumo. Ma sorge un interrogativo: perchè non tutti i fumatori si ammalano di queta malattia? Ci sono ipotesi su una possibile predisposizione genetica e sul fatto che potrebbe essere il risultato di meccanismi autoimmuni, innescati dal tabacco.
Quali sono i sintomi del morbo di buerger?
La sintomatologia è abbastanza chiara: il paziente avverte dapprima dolori o formicolii al polpaccio, facile stancabilità dell’arto, quindi claudicatio intermittens, cioè necessità in piena deambulazione di arrestarsi per violenti dolori alle gambe determinati da mancanza di afflusso sanguigno ai muscoli e, in fase avanzata, per violentissimi dolori anche a riposo e disturbi trofici che vanno dal pallore della pelle alla formazione di chiazze di colore vinose, ad alterazioni fino a giungere , nei casi più gravi alla gangrena.
Diagnosi
La diagnosi della morbo di Buerger richiede un’approfondita anamnesi, in cui viene richiesto al paziente il rapporto con il fumo. A seguire il medico procederà ad un accurato esame obiettivo, concentrato sulle estremità (gambe e braccia, ma soprattutto mani e piedi).
Potrebbero venire richiesti i seguenti esami:
- Ecografia
- ABI (ankle brachial index), un esame fondato sulla valutazione delle differenze di pressione del sangue tra diverse arterie della gamba
- Tomografia computerizzata (TC)
- Angiografia
- Angiografia a risonanza magnetica
La malattia di Buerger è diagnosticata per esclusione, ossia solo dopo aver scartato eventuali patologie aterosclerotiche, emboli, malattie autoimmuni, trombofilia e diabete mellito
Come si cura il morbo di beurger?
La cura può essere vasodilatatrice, iodico-vitaminica, integrata da una cura fisioterapica e chirurgica (asportazione dei gangli nervosi). E’ comunque indispensabile la sospensione immediata del fumo.
