Sclerosi multipla:sintomatologia
I sintomi tipici sono il risultato dell’ interessamento delle più lunghe fibre mieliniche del sistema nervoso centrale (sintomi visivi, piramidali, sensibili, o cerebellari). Rari sono invece i sintomi da interessamento corticale (ad esempio afasia, aprassia). La sclerosi multipla è una malattia altamente imprevedibile, sia per decorso clinico sia per prognosi. Caratteristica fondamentale di questa malattia è l’estrema variabilità focale, temporale e spaziale, con cui essa si manifesta.
I sintomi più comuni all’esordio sono compromissione delle:
• funzioni visive
• parestesia e disestesie
• sintomi cerebellari
• diplopia
• vertigini
• disturbi della minzione
• disturbi sessuali
• stipsi
• manifestazione parossistiche o psichiche
Il corredo sintomatologico è alquanto proteiforme, perché le lesioni hanno estrema variabilità focale e temporale. Per quanto attiene alle funzioni piramidali di fronte un paziente con sospetta SM vanno ricercati eventuali segni e sintomi con l’esame neurologico anche perché a volte il paziente non ha sintomi (deficit di forza) ma spesso presenta dei segni ipereflessia, segno di Hoffmann di Babinski, assenza dei riflessi esterocettivi. La manifestazione tipica è l‘astenia, cioè il deficit di forza che può andare fino alla plegia e interessare un arto isolato (monoplegia) e due arti (emiplegia e paraplegia). Altro disturbo è la spasticità. In alcuni casi possono manifestarsi degli accessi di spasmi tonici tipicamente notturni e dolorosi. Altro sintomo molto frequente è la fatica, tra l’altro notevolmente influenzata dalla temperatura corporea (miglioramento con il freddo).
La spasticità, legata all’ aumento del tono muscolare da lesione delle vie cortico-spinali, è un sintomo molto frequente, di intensità variabile, talora fluttuante nello stesso paziente. In alcuni casi possono manifestarsi degli accessi di spasmi tonici, tipicamente notturni e dolorosi. La spasticità oltre a interferire con il fisiologico movimento degli arti, se grave e non trattata, può rendere problematico il mantenimento della posizione seduta, i trasferimenti, le manovre igieniche e infine condurre ad alterazioni tendinee con anchilosi delle articolazioni interessate.
La fatica è uno dei sintomi più comuni nella sclerosi multipla, interessando circa 85% dei pazienti ed è uno dei sintomi più disabilitanti. Si può definire come senso opprimente di stanchezza, mancanza di energia e sensazione di spossatezza esagerata rispetto all’effettivo livello di attività esercitata. Può comparire in qualsiasi stadio della malattia e non è strettamente correlata alla disabilità e la compromissione delle funzioni piramidali. Meccanismi diversi influiscono su questo sintomo: una scarsa qualità del sonno, legate alla nicturia, agli spasmi notturni o al dolore; fattori immuno-correlati tra cui le citochine pro-infiammatorie; l’uso di farmaci soprattutto beta-interferon e antispastici; depressione; aumento dell’utilizzo di energia legata alla debolezza e alla spasticità. Essa inoltre è notevolmente influenzata dalla temperatura corporea; molti pazienti riferiscono un soggettivo miglioramento della loro condizione clinica con il freddo.
Funzioni visive. Le anomalie del sistema visivo sono assai frequenti, potendo in molti casi rappresentare l’unico sintomo all’esordio. Solitamente la regressione di tale sintomatologia è pressoché completa. Il paziente lamenta tipicamente una sensazione di calo della vista quasi sempre unilaterale che si sviluppa in alcuni giorni, con sensazione di “vedere appannato“, “attraverso una tendina” o “un vetro smerigliato“: questa condizione è spesso legata ad un interessamento del nervo ottico, l’unico nervo periferico la cui mielina è formata per intero dall’ oligodendroglia, denominato neurite ottica retrobulbare (NOR); spesso è preceduta da dolore retro o sovraorbitario. L’esame del campo visivo evidenzia uno scotoma centrale o centrocecale. In alcuni casi solo lo studio dei potenziali evocati visivi può evidenziare una neuropatia ottica latente. Questi pazienti possono sviluppare un temporaneo calo di visus, in seguito a uno sforzo o aumento della temperatura corporea (fenomeno di Uhthoff).
Funzioni cerebellari. I peduncoli cerebellari ed il cervelletto rappresentano sedi tipiche per lo sviluppo di placche di demielinizzazione. Il sintomo maggiormente frequente è l’atassia, statica e dinamica, che nelle forme più gravi interessa anche gli arti inferiori e il tronco. Nelle forme avanzate di malattia è caratteristica l’andatura atasso-spastica, per coinvolgimento contemporaneo del sistema piramidale e cerebellare. Inoltre sono spesso presenti dismetria e adiadococinesia agli arti superiori e soprattutto tremore intenzionale con oscillazioni di grande ampiezza che influenzano notevolmente la normale gestualità; il tremore può interessare anche capo e tronco, essere presente anche a riposo ed assumere proporzioni violente col movimento (tremore posturale).
Funzioni del tronco encefalico. Un sintomo molto tipico è la diplopia, per interessamento dei nervi dell’oculomozione, ed in particolare dell’abducente o del fascicolo longitudinale mediale: viene infatti descritta come caratteristica peculiare della sclerosi multipla l’olftalmoplegia internucleare. Altro segno caratteristico è la paralisi facciale, spesso di tipo periferico, insorgente talora all’esordio della malattia. Nelle forme più gravi vi può essere un danno dei nervi cranici bulbari o dei trattti cortico-bulbari con disfagia, disfonia e disartria, che in alcuni casi può assumere le caratteristiche della paralisi pseudo-bulbare, associandosi a crisi di riso e pianto spastico e deterioramento intellettivo. Sebbene la frequenza di vertigini franche sia modesta (20% circa) è molto tipico all’interessamento del sistema vestibolare, che si manifesta con alterazioni dell’equilibrio alla stazione eretta e alla deambulazione.
Funzioni sensitive. Le anomalie della sensibilità sono molto caratteristiche; le più frequenti sono legate al danno delle vie lemniscali lungo il midollo con manifestazioni parestesiche ed alterazioni della pallestesia agli arti, soprattutto inferiori con atassia. Un sintomo caratteristico è rappresentato dal segno di Lhermitte caratterizzato da una sensazione di scossa elettrica che si propaga lungo la colonna agli arti inferiori e talvolta agli arti superiori.
Funzioni sfinteriche e viscerali. Approssimativamente il 90% dei pazienti con SM accusa disfunzioni sfinteriche urinarie o fecali o disfunzioni sessuali. La manifestazione più tipica è rappresentata dalla instabilità e iperreflessia detrusoriale. I pazienti avvertono l’anomalia come urgenza minzionale, talora associata a una vera e propria incontinenza da urgenza e spasmi dolorosi alla vescica. Altre alterazioni urologiche includono la dissinergia vescico-sfinterica. Nelle forme più severe e più datate di malattia i pazienti possono andare incontro ad una areflessia detrusoriale. I problemi sessuali sono altrettanto comuni.
Nell’uomo sono tipici il deficit di erezione, l’aumento della soglia di percezione dello stimolo sensitivo durante l’atto sessuale. In ultimo anorgasmia ed alterazioni dell’eiaculazione, in particolare eiaculazione retrograda. Nella donna spesso si possono incontrare disestesie dell’area perineale, riduzione della sensibilità perineale. In entrambi i sessi, può inoltre associarsi riduzione della libido, spesso su base psicogena, con conseguente aumento dei problemi della sfera sessuale.
I disturbi del tratto gastrointestinale sono rappresentati dalla stipsi ed in minor misura diarrea e incontinenza. La costipazione può essere correlata a numerosi fattori: in particolare si nota una riduzione del riflesso gastro-colico che normalmente si evidenzia con uno stimolo alla defecazione in seguito ad un pasto abbondante, soprattutto ricco di fibre carnee e grassi; possono giocare un ruolo anche i farmaci eventualmente assunti dai pazienti.
Funzioni mentali. Le manifestazioni da interessamento delle funzioni superiori sono molto frequenti: si calcola che circa il 50% dei pazienti accusi, nel corso della malattia, un progressivo declino cognitivo. Una demenza grave si manifesta in circa il 20% dei pazienti. Le manifestazioni più tipiche sono quelle della demenza sottocorticale, con perdita della memoria di fissazione e, quindi, a lungo termine del mantenimento dell’attenzione e della fluenza verbale. In altri casi si può evidenziare il quadro psico-cognitivo della demenza da interessamento prefrontale o della sindrome pseudo-bulbare.
Dolore. Molti pazienti accusano sintomatologia dolorosa, che si desume essenzialmente nel dolore cronico della colonna spinale, in particolar modo a livello dorso lombare, talora legato alle anomale posture assunte, soprattutto dai pazienti costretti sulla sedia a rotelle.
